I understand that if I do not cancel my subscription, I will be charged every 3 months using the payment information that I have provided. Specifically, I acknowledge that it is not the responsibility of Carlos K. Wesley MD PC to cancel my subscription for me.
While Carlos K. Wesley, MD PC will gladly cover the shipping charges for items returned in good condition, a restocking fee of 20% for returned items that a customer has deemed in excess of their need.
